The capitated revenue model demands a wrap-around approach to patient care, with primary care being a preventative tool to avoid complicated and expensive inpatient procedures.

 

The focus upon geriatric populations similarly requires frequent visits, communication with patients and ease of access to care. Typical clinics are in the range of 9,000-12,000 SF and include services such as phlebotomy and a dispensing pharmacy. Originally based in Chicago, Oak Street Health operates in multiple markets around the US including Chicago, Detroit, Indianapolis and Philadelphia.
